Social Security Application
Apply for a Social Security number if you have a job offer or an assistantship and have been in the U.S. for 10 days or more. 

Checklist

• Original job offer letter from the department offering employment or assistantship support letter. The letter must be on department letterhead and include your wages (see sample letter HERE).
• Original supporting letter from International Programs and Services on department letterhead.
• Completed and signed application for a Social Security Card (form SS-5 HERE).
• Original plus one photo copy of:
• 1-20 or DS-2019
• Passport
• Visa
• I-94

Take completed application, your job offer letter or assistantship support letter, the International Programs and Services supporting letter, and immigration documents (plus photocopies) to the Social Security Administration.
